Supervise and coordinate workers engaged in assessing individual needs to delay or prevent inappropriate institutionization. Customers (veterans) will meet with a multidisciplinary team of health professionals that conduct a comprehensive assessment of each potential participant to determine which Health Care services meet the individual's specific health and social needs. Each participant is provided an individualized plan of care, which include either nursing, medical and/or social services as well as occupational, physical and/or speech therapy amongst other services. Train customer service personnel on reviewing the health professionals' order and in interpreting the recommendations to the customer as well as in the resolving of customers' complaints. Also train employees on company policies and on how to handle customer service calls and resolve problem. Answer customer questions & investigate & correct errors, following customer and establishment records. Conflict resolution. Monitor calls. Keep records of complaints in computer and inform appropriate department of complaints against them. Recommend employees for reprimanding and/or firing of employees. 2 yrs. college + 2 yrs. exp. 40 hrs/wk.
